Abscesos retrofaríngeos
¿Qué es un absceso retrofaríngeo?
Un absceso retrofaríngeo es una infección que se forma detrás de la pared posterior de la garganta. No son frecuentes en niños, pero cuando aparecen, suelen ocurrir en niños menores de 5 años.
¿Cuáles son las causas de los abscesos retrofaríngeos?
Los abscesos retrofaríngeos ocurren cuando las bacterias crecen dentro de los ganglios linfáticos que hay detrás de la garganta. Los ganglios linfáticos ayudan a combatir las infecciones eliminando los gérmenes, pero a veces no lo consiguen.
Se puede formar un absceso después de que un niño tenga una infección de oído, de garganta o de los senos paranasales. También pueden ocurrir después de una herida en el cuello o en la garganta.
¿Cuáles son los signos y los síntomas de un absceso retrofaríngeo?
Al principio, los abscesos retrofaríngeos pueden causar dolor de garganta, fiebre y dolor en los ganglios linfáticos del cuello.
Si no se tratan, los síntomas pueden empeorar. Un niño con un absceso retrofaríngeo puede:
- babear
- negarse a comer o a beber debido al dolor al tragar
- tener rigidez de cuello y no quererlo mover por el dolor
- tener el cuello o los ganglios linfáticos del cuello hinchados
- mantener la boca bien cerrada
- hablar con voz ronca o apagada
- tener problemas para respirar o hacer ruido al respirar
- tener dolor en el pecho
¿Cómo se diagnostican los abscesos retrofaríngeos?
Los médicos suelen diagnosticar los abscesos retrofaríngeos basándose en los síntomas y en la exploración de la boca, la garganta y el cuello del niño.
Si este diagnóstico parece probable, el niño debería visitar a un médico especializado en el oído, la nariz y la garganta (un otorrinolaringólogo). Los análisis de sangre, las radiografías y las tomografías computadas (TC) pueden ayudar a los médicos a hacer el diagnóstico.
¿Cómo se tratan los abscesos retrofaríngeos?
Los niños con abscesos retrofaríngeos deben recibir tratamiento en un hospital. Los médicos les administrarán antibióticos por vía intravenosa (VI) y es posible que tengan que operarlos para drenar el absceso. Esto será recomendable si:
- El niño tiene problemas para respirar.
- El absceso es muy grande.
- Los síntomas no mejoran después de uno a dos días de tratamiento con antibióticos.
Si un niño tiene problemas para respirar, los médicos usarán un tubo endotraqueal para mantener abiertas las vías respiratorias antes de operarlo.
La mayoría de los niños pueden comenzar a tomar antibióticos orales (tomados por la boca) si sus síntomas mejoran después de varios días de antibióticos por vía intravenosa (VI).
¿Cuándo debería llamar al médico?
Llame al médico si su hijo tiene dolor de garganta y fiebre. Busque ayuda médica inmediata si su hijo:
- tiene fiebre alta
- está babeando y no consigue tragar
- tiene rigidez en el cuello
- tiene dificultad para respirar
