¿Qué son los cuidados paliativos?
Los cuidados paliativos ofrecen apoyo físico, emocional y espiritual a los niños enfermos y a sus familias. A través de un equipo médico, que incluye médicos, especialistas en el tratamiento del dolor, personal de enfermería, trabajadores sociales y terapeutas, los cuidados paliativos ayudan a prevenir y aliviar el dolor y el sufrimiento, así como a reducir el estrés, la ansiedad y el miedo asociados a las enfermedades graves.
Los cuidados paliativos no se deben confundir con los cuidados para enfermos terminales, destinados a aquellos pacientes que no se espera que se recuperen. El objetivo de los cuidados paliativos consiste en mejorar la calidad de vida del niño y sus familiares durante una enfermedad grave, y en ayudar a las familias a tomar decisiones importantes sobre el cuidado del niño.
¿Quién necesita recibir cuidados paliativos?
Cualquier niño que tenga una enfermedad grave, compleja o de riesgo vital (independientemente de que se espere que tenga una recuperación total, que viva siempre con una enfermedad crónica o que muera debido a la afección que padece) puede ser un candidato a recibir este tipo de cuidados.
Los cuidados paliativos ayudan a niños que están afrontando enfermedades como el cáncer; afecciones neurológicas; y enfermedades cardíacas, pulmonares, renales o hepáticas (del hígado).
¿En qué se diferencian los cuidados paliativos de los cuidados para enfermos terminales?
Mucha gente confunde los cuidados paliativos con los cuidados para el final de la vida. Los programas de cuidados para enfermos terminales y los de cuidados paliativos comparten la meta similar de aliviar los síntomas y controlar el dolor. Pero no son lo mismo:
- Los cuidados paliativos pueden ocurrir en cualquier etapa de la enfermedad, incluso desde el momento del diagnóstico. No dependen del pronóstico (cómo se prevé que evolucionará el paciente) y se pueden ofrecer junto con los cuidados curativos o para alargar la vida. Los cuidados paliativos se pueden proporcionar en hospitales, en centros ambulatorios y en el domicilio del paciente.
- Los cuidados para enfermos terminales son un tipo específico de cuidados paliativos. Se centran en brindar cuidados a pacientes que no espera que se recuperen. Están destinados a aquellas personas que han dejado de recibir tratamiento para su afección médica y cuya esperanza de vida no supera los 6 meses. Estos cuidados se pueden proporcionar en el domicilio del paciente, en el hospital o un centro para enfermos terminales.
¿Qué servicios se ofrecen?
Los cuidados paliativos están pensados para satisfacer las necesidades exclusivas de cada niño y de cada familia. Las familias pueden adaptar los cuidados del niño a sus propias necesidades y recibir tanta o tan poca ayuda como deseen.
El equipo de cuidados paliativos puede estar integrado por:
- médicos especializados en cuidados paliativos, que proporcionan cuidados médicos directos y manejan el tratamiento de síntomas físicos, como el dolor, las náuseas, el cansancio, la falta de aire y los problemas para dormir
- personal de enfermería, que evalúa las necesidades complejas del paciente y de la familia, facilita la comunicación con el equipo médico y dentro de la familia, y coordina los cuidados entre los distintos miembros del equipo médico
- trabajadores sociales, que ofrecen asesoramiento, ayudan a las familias a entender mejor el sistema sanitario, brindan información sobre otros recursos y se encargan de las derivaciones a otros profesionales
- especialistas en vida infantil, que abordan las necesidades evolutivas del niño, ayudan a que entienda lo que le pasa y a que participe en la toma de decisiones cuando le corresponda, al tiempo que ayudan a mantener una sensación de normalidad
- fisioterapeutas y terapeutas ocupacionales, que ayudan a que los niños estén más cómodos y les enseñan a usar mejor sus habilidades físicas
- arteterapeutas y musicoterapeutas, que ayudan a que los niños exploren y expresen sus sentimientos a través de la música, el arte, la poesía y otras formas creativas de expresarse
- capellanes y otros consejeros espirituales, que ofrecen apoyo y asesoramiento relacionados con las creencias religiosas; también pueden ofrecer asesoramiento para el duelo
- terapeutas masajistas, que favorecen la relajación, ayudan a los pacientes y a sus familias a manejar el estrés, y ayudan a aliviar al dolor
- dietistas o nutricionistas, que organizan planes de alimentación saludables y hacen recomendaciones para una alimentación nutritiva
- asistentes de salud a domicilio, que ayudan a los pacientes a hacer la transición del cuidado hospitalario al cuidado en el hogar, y ayudan en las necesidades de atención sanitaria que tengan los pacientes en su domicilio
Por lo general, cada hospital tiene su propio tipo de equipo de cuidados paliativos. Algunos equipos hasta incluyen voluntarios de la comunidad que quieran ayudar de la forma en que puedan.
¿Quién paga los cuidados paliativos?
La mayoría de los planes de seguros médicos cubren la totalidad o parte de los cuidados paliativos y muchos servicios de cuidados paliativos cuentan con el respaldo de donaciones benéficas. Si el costo de los cuidados paliativos es un motivo de preocupación, un trabajador social lo puede asesorar sobre cómo cubrirlo.
Los cuidados paliativos, ¿son adecuados para su hijo?
Decidir si las necesidades de su hijo podrían ser satisfechas por los servicios de cuidados paliativos es una decisión personal que solo puede tomar después de haber hablado con el profesional de la salud que lo atiende, considerado las necesidades individuales de su hijo y de su familia, y determinado los servicios disponibles en su área.

¿Cómo puedo ayudar a mi hijo?
Si su hijo se beneficiaría de un programa de cuidados paliativos, recuerde que esos servicios también lo ayudarán a usted. Los familiares que cuidan de niños gravemente enfermos deben afrontar muchos desafíos y los programas de cuidados paliativos pueden ayudar a aliviar en parte esta carga.
Los padres y otros cuidadores también desempeñan un papel importante en la salud de un niño. Usted es quien más conoce a su hijo, de modo que trabajará junto con el equipo de cuidados paliativos. Todos tienen la misma meta: proporcionar a su hijo los mejores cuidados.
